REQUIREMENTS FOR REGISTRATION

·         PROOF OF RESIDENCE:  A paid utility bill (gas or electric) with correct name and address, Lease or Rental Agreement, Settlement Statement if you have just purchased the house or Earnest Money Contract with approval letter from Mortgage Company if you have not closed on your house yet.

·         BIRTH CERTIFICATE

·         CHILD’S SOCIAL SECURITY CARD (If not available, your child          will be assigned a state ID number.)

·         PARENT’S DRIVER’S LICENSE

·         IMMUNIZATION REQUIREMENTS: KG/1^st Grade

DPT/DTaP – Five doses for children 4 – 6 years: if 4^th dose was given on or after 4^th birthday, then a 5^th dose is not necessary.  Three doses are required for children 7 years and older with at least one dose on or after the 4^th birthday.

Polio (IPV) - Four doses, one since the 4^th birthday, or if 3^rd dose was given on or after 4^th birthday, only three doses required: 5^th dose required if 4^th dose was given before 4^th birthday.

MMR – Two doses with the 1^st dose on or after the 1^st birthday: 2^nd dose – at least 30 days apart from the 1^st dose

                      Hepatitis A (HAB) - Two doses

Hepatitis B (HBV) – Three doses

Varicella (Chickenpox) – Two doses with the first dose on or after the 1^st birthday, or parent/physician statement of chickenpox illness

Having copies already made of your child’s birth certificate, immunization records, social security card and your proof of residence will save considerable time for you on the day of registration.  Please bring originals for verification.
