See the attached athletic packet which contains important information on Katy Independent School District Athletic Policies and Procedures. It also contains important health and safety information for all student athletes and their parents.

• All required forms (also attached) must be completed in their entirety and returned to the athlete’s head coach or athletic trainer before a student participates in any try-out, practice, athletic class, open gym, open weight room, athletic competition, or travels with an athletic team for any purpose. 
• The student is required to use the Pre-participation - Physical Examination Form enclosed. NO OTHER Physical Examination Form can be accepted as per the U.I.L. A new physical exam must be given prior to each school calendar year. Any physical administered prior to May 1, may not be valid after August 1.

Please read and print a copy of the Student/Parent Athletic Packet for your records. When you sign the required forms, you are agreeing to abide by all stated policies and rules.
